Tools for Programming

Provided in this section will be a variety of resources and tools which will be helpful in implementing Caregiver services in your community.  AoA is in the process of obtaining good examples to assist you in your work and additional items will be added to this site in the future.   Assessment instruments, one example of a tool for programming, are the first step in judging the needs of caregivers in your communities and essential to providing services that meet those needs.  Following are examples of three caregiver assessment instruments.

Caregiver Assessment Screen/Caregiver Assessment Tool (Canada)

A good set of tools developed in Canada by Nancy Guberman, Janice Keefe, Pamela Fancey, Daphne Nahmiash, and Lucy Barylak. First, a brief Caregiver Assessment Screen that can be used by a home care agency to determine whether a caregiving situation is "at risk" and the level of urgency required for intervention. This takes about 15 minutes to administer and can be used over the telephone. Secondly, a comprehensive Caregiver Assessment Tool designed to collect information on many different areas of a caregiver's situation, and to pinpoint from this information the key areas of difficulty being experienced and the types of services or support that would best assist the caregiver. This usually takes about an hour and one half to administer. Caregiver Assessment Forms and Instructions (PA Department on Aging)

Pennsylvania Department on Aging’s Caregiver Assessment Forms and Instructions [PDF version] - This material, which was handed out at AoA’s 2000 Symposium "Building an Aging Network Toolkit", is part of PA’s Comprehensive Options Assessment instrument.

Caregiver Assessment Tool and Instruction Manual (California)

California Caregiver Assessment Tool and Instruction Manual - The CA assessment materials have been developed by the Family Caregiver Alliance and are used by the state's Caregiver Resource Centers to examine the needs of families caring for adults who suffer chronic brain disorders, e.g. Alzheimer’s disease, stroke, Parkinson’s disease, traumatic brain injury, etc. FCA does thorough training for all staff that conduct caregiver assessments. Those of you who want a copy of these materials, please send a request to Lana Sheridan, Family Caregiver Alliance, National Center on Caregiving, 690 Market Street, Suite 600, San Francisco, CA 94104. Your request should include a check for $25.00 to cover handling, printing, and shipping costs.
