Grantee Reporting Requirements

Formula Grants – Title III State Program Report (SPR) and Title VII National Ombudsman Report (NORS)

Title III grants require the submission of the State Program Report (SPR) for its GPRA output and outcome data. Instructions for submission of SPR data can be found here.

Title VII - The State Long-Term Care Ombudsman Program requires use of the National Ombudsman Report System (NORS) Data Input Application for electronic submission of its GPRA output and outcome data. Instructions for NORS data can be found here.
