Grantee Reporting Requirements
Formula Grants - Standard Form 269 (SF-269)
AoA’s formula grants have a one year project period however
an additional two years to expend these funds is allowed because
of the hierarchical structure of the aging network’s funds
distribution system. Standard Form 269 (SF-269)
With semiannual financial reporting for each of the three years
that the grant remains active, and a final report due 90 days
after the project end date, a total of seven financial status
reports are required for each formula grant.
The first Financial Status Reports is to be submitted within 30
days after the end of the first six months.

Final SF- 269
The final SF – 269 Financial Status Report is due 90 days
after the 17th month, which is the project end date for the total
three year project period.
This report forms the basis of the close out of the grant. Any
project funds that remain unused at the end of the budget period
as reported on the final SF-269 are returned to the Treasury,
unless the grantee has requested an extension of the time available
to expend the funds during the time the grant was active.
