Non-Discretionary (Formula/Mandatory) Grants - Standard Form 425 (SF-425)
AoA’s non-discretionary (formula/mandatory) state grants have a one year project period however an additional two years to expend these funds is allowed because of the hierarchical structure of the aging network’s funds distribution system.
A Federal Financial Report (SF-425) is due semi-annually. Reports are due within 30 days of the end of the 2nd and 4th quarters of the Federal fiscal year (April 30 and October 30).
In addition, a Federal Financial Report AoA Supplemental Form to SF-425 for the Title III program is due semi-annually.
With semi-annual financial reporting for each of the three years that the grant remains active, and a final report due 30 days after the project end date, a total of six financial status reports are required for each formula state grant.

This report forms the basis for the close out of the grant. Any project funds that remain unused at the end of the budget period as reported on the final SF-425 are returned to the U.S. Treasury.
