Hispanic Elders Project
The Purpose of the Program
To support the efforts of communities to improve the health of Hispanic Elders, AoA, AHRQ, HRSA, CDC and CMS launched an initiative in 2007 involving the metropolitan areas of Chicago, Illinois, Houston, Texas; Los Angeles, California, McAllen, Texas; Miami, Florida; New York, New York; San Antonio, Texas; and San Diego, California. Under the leadership of the Area Agency on Aging, each of these communities formed broad-based coalitions to develop coordinated strategies for improving Hispanic elders' health. HHS supported these communities through on-going technical assistance, specialized data analysis, and a collaborative learning network dedicated to developing multiple approaches which address health disparities.
Each community team developed and implemented a plan for addressing one or more health disparities affecting communities of Hispanic elders, and many teams identified diabetes as a primary issue of concern for this population. Adoption of a diabetes self-management program was central to each team’s approach. Self-management programs empower individuals to take control of their health by engaging in self-management strategies that address behavior change.
Although funding for the Network has ceased, the project sites are still active and engaged in improving the health of Hispanic older adults. A report from this collaborative with summaries of each grantee’s activities is soon to be released by the Agency for Healthcare Research and Quality.

Funding History
AoA began supporting the Hispanic Elders Project in 2005 by awarding $200,000 in technical assistance to support a learning network of communities to launch diabetes education initiatives for Hispanics over 60. In 2008, AoA provided another $200,000 to support these community initiatives.
