As part of the Alzheimer's Demonstration Program, numerous products have been
created to assist professionals and providers in working with Alzheimer's patients.
These products are identified here by type of material and format. The descriptions
include information regarding how and where to obtain each product. The indexes
list the products by the target audience for which they were intended.
Contact information for the Alzheimer's Demonstration projects is presented here,
both for previous and for current grantees. Project contacts can answer specific
questions about the materials and programs that they developed.
In this section, we have also included specific data and reports resulting from
a national evaluation of the Alzheimer's Demonstration Project. A new evaluation
report is expected to be released by the end of 2001.
