Hand In Hand: A Start-Up Kit for Dementia Day Services

Description
This 200-page manual covers the essential topics for starting an adult day services program specifically for people with dementia. The Kit includes sample forms (e.g., sample intake, care plan, client discharge, referral log), admissions agreement, and job descriptions on computer disk. Samples of charting progress notes and vignettes on dealing with common behaviors in the adult day care setting are provided along with many other central topics. The Start-Up Kit is very useful for orienting staff, for developing new programs, and as a general reference manual.
